Payment Policy

We accept cash, checks, Visa, Mastercard, American Express, Debit Cards and HSA cards.

We offer a 10% prompt pay discount for patients with no insurance or those choosing not to bill their insurance and will gratuitously bill for those with coverage, so the payment will go directly to you.

We provide Good Faith Estimates for self pay (cash-uninsured) and insured patients opting to not use their insurance.

For your convenience, we’ve provided all of our patient forms below. New patients are encouraged to download, print and fill out the forms and bring them in the office with you when you arrive for your appointment.
